Preliminary Program Enhancements (from “Key Recommendations” page 13 River Communities Health Assessment July-September 2007):

1. 1. Primary Care
   • Add two physician level primary care providers:
     One hired with a start date of August 2008
     Recruitment for second provider underway
   • Add one Nurse practioneer:
     Hired
2. 2. Clinical Preventive Services
   • Comprehensive program needs to be developed; may include expansion of Diabetes program to general Nutritional program;
   • Add educational programs targeting specific health issues
   • Program specifics should be developed following presentations to communities
3. 3. Resident Satisfaction
   • Publicize results
   • Investigate on-going monitoring program for all patient contact areas
4. 4. Focused Community Outreach
   • Review needs with communities
   • Investigate creation of screening and educational programs
5. 5. Partnership Development
   • Cardiology established with St. Joseph’s Syracuse
   • Nephrology proposed
   • Oncology under consideration
