Update 2008: Standards in Diabetes Medical Care

River Hospital’s Nutrition Services will present ‘Update 2008:  Standards in Diabetes Medical Care’.  This presentation is an outreach service designed for Health Care Providers, Physicians, and Care Givers and will be presented by Nicole Garnsey, Registered Dietitian and Certified Diabetes Educator.  You will hear the latest American Diabetes Association recommendations in screening for diabetes, monitoring and achieving glycemic goals, clinical preventative services, and counseling patients.   While this is opened to health care professionals, care givers, and individuals with diabetes, there will be valuable information for everyone.

                               

This event is scheduled for Thursday, September 4th at River Hospital’s Adult Day Health Care Room beginning at 6:00 p.m. The presentation will be approximately one hour and is open to the public. Light food and beverages will be provided and seating is limited to 30 people.  RSVP by August 28th to reserve your seat by calling 783-6810.

Adult Day Health Care

Did you know that River Hospital offers a Medical Model Adult Day Health Care Program??

The program is a medical, community-based, ambulatory service which promotes individual wellness to elderly and disabled adults.  River Hospital is one of two locations in the Jefferson and St. Lawrence County area offering this medical model program.

Adult Day Health Care offers registrants comprehensive health care in a congregate day setting designed to meet individual needs of chronically ill, frail, elderly and disabled adults requiring certain primary preventative, diagnostic, therapeutic, rehabilitative or palliative services on an outpatient basis.
Registrants are provided transportation to and from the program, where their needs are assessed and met through an individualized coordinated plan of care.  Each plan is developed and implemented with the oversight and consultation of a professional medical team to ensure all health needs are addressed.  This type of program decreases the number of medical emergencies by having the Adult Day Health Care team observe, identify and act upon medical issues before the issues become more serious.

While this outpatient program is designed for elderly and dependent adults, it also offers ongoing relief for the caregiver for as many as five days each week.  This option for day time care affords the primary caregiver the opportunity to work during the day, the ability to tend to personal needs and appointments, or simply offer a respite from the arduous task of round-the-clock care for a dependent loved one.

This medical and social service provides the needs for dependent individuals, allowing them to remain at home and part of the community longer.  This program contributes to quality of life for the participant through quality, stable healthcare in the home community.
